The Delaware Department of Insurance published a Bulletin informing all insurers, brokers, and agents writing auto insurance in the state on the requirement of the use of certain forms. Starting 90 days from the publishing of the Bulletin (4/15/2024), the use of these forms will be mandated, and no other format for the required information will be accepted. The three forms are available on the Delaware DMV website.

FR1–Delaware Uninsured Vehicle Report is the form insurers must use to report any person involved in an auto accident who does not have auto insurance to the DMV. The insurer should report the person’s name, address, and description of the vehicle.
